We encourage you to continue your care with Pathways at St. Francis Primary Care Clinic in Leesburg, where you will receive the same high-quality care and attention. St. Francis Primary Care Clinic is dedicated to providing excellent medical services and will be able to provide you with the same high-level care that you have come to expect.

Additionally, we are pleased to offer transportation services to St. Francis to make this transition as smooth as possible for you. If you need assistance with transportation, please let us know, and we will coordinate the details for you.

Cancellation of Pending Appointments

Please note that all scheduled appointments after April 30, 2025 at CareFirst Family Practice Belleview will be cancelled. If you would like to reschedule with St. Francis, please call St Francis Primary Care Clinic (352) 314-6589.

Accessing Your Medical Records

Your medical records are important for ensuring continuity of care. To access your medical records, please follow these steps:

1. Contact our office at 800-632-6228 ext 2 to request your records with our Billing Department.
2. Confirm your information and they can be sent to you or faxed directly to your new Primary Care Provider.
3. Leave a message with your first name, last name, and phone number if nobody answers your call. We will respond to you as soon as possible.
